Quartz operates as a joint venture between Gundersen Health System, UW Health, UnityPoint Health–Meriter, and Physicians Plus Insurance Corporation since 2017.
Quartz provides ACA-compliant health plans across Wisconsin, Illinois, Minnesota, and Iowa with over 350,000 members as of 2023. Quartz offers HMO and point-of-service (POS) plans with access to networks such as Quartz Direct and Unity Network in southern and central Wisconsin.
Members use the MyChart app for claims tracking, provider search, and digital ID cards, YourInsurance.info states. Quartz Medicare Advantage plans include options like Quartz Elite with out-of-pocket maximums capped at $5,900 for 2024.
Employers select group coverage options that meet federal requirements under the Affordable Care Act for both large and small businesses. Individuals apply for Quartz insurance on the Healthcare.Gov marketplace or via an employer’s group enrollment portal.
